Privacy And Collection Statement For Clients

This privacy statement is to provide information to Swindon Sports Therapy clients as to how your personal information (including health information) is collected and used, and the circumstances in which we may share it with third parties.

When you register as a client of Swindon Sports Therapy, you provide consent for our practitioners and staff to access and use your personal information so they can provide you with the best possible healthcare. Only those who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you to do this.

Swindon Sports Therapy will need to collect your personal information to provide healthcare services to you. Our main purpose for collecting, using, holding and sharing your personal information is to manage your health.

Information Collected

Identity data – Details about you, such as your name, DOB and address.

Contact data – Email address and contact number – we will use this data to contact you about your appointment and send you information specific to an appointment you have booked. If you opt in to receive our newsletter you can unsubscribe at any time by clicking on the link at the bottom of a newsletter email sent to you or by contacting us directly.

Medical data – Relevant to your treatment, including medical history, medications, allergies, adverse events, social history, family history and risk factors

With your permission we may send you exercise video tutorials to help with your recovery. This is done through a secure exercise library app. We are required to enter your name and email address. You are then sent a link to access your pin protected portal of exercises.

Swindon Sports Therapy will collect your personal information:

  1. When you make your first appointment
  2. During the course of providing healthcare services, we may collect further personal information.
  3. We may also collect your personal information when you send us an email or SMS, telephone us, make an online appointment or communicate with us using social media.
  4. In some circumstances, personal information may also be collected from other sources. Often this is because it is not practical or reasonable to collect it from you directly. This may include information from:
  • Your guardian or responsible person.
  • Other involved healthcare providers, such as specialists, allied health professionals, hospitals, community health services and pathology and diagnostic imaging services.

Information We May Share

We will only ever share your information if you specifically request us to do so. This may involve sharing your information with:

  • Other healthcare providers.
  • Insurance companies as part of a legal claim.
  • When it is necessary to lessen or prevent a serious threat to a client’s life, health or safety or public health

Confidentiality and Privacy

Only people that need to access your information will be able to do so. Other than in the course of providing healthcare services or as otherwise described in this policy, Swindon Sports Therapy will not share personal information with any third party without your consent.

Swindon Sports Therapy will not use your personal information for marketing any of our goods or services directly to you without your expressed consent. If you do consent, you may opt-out of direct marketing at any time by notifying us directly.

Our practice stores all relevant information on our purpose built app. It is password protected and is never shared with anyone. From the 29th May 2023 we will be switching to Power Diary as our practice management software which exceeds all security standards.

We store all data indefinitely as it may be relevant if you return to the clinic at a later date or we need to send your notes onto your GP or for insurance purposes. We are legally obligated to retain this information for 7 years.

You have the right to request access to, and correction of, your personal information.

Your Right to Access

Swindon Sports Therapy acknowledges clients may request access to their health records. We require you to put this request in writing, in person, mail or email. All requests must be accompanied by photo ID. Our practice will respond within 10 working days.

Swindon Sports Therapy will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time-to-time, we will ask you to verify your personal information held by our practice is correct and up to date.
